When rectangular wire is utilized for the purpose of correcting a crossbite, the twist applied to produce the desirable amounts of crossbite correction too often results in the orthodontist concentrating on a rotational direction of the moment without the awareness that forces involved in producing the corrective moment are (vertical).

It requires two vertical forces to twist this wire and place it into the bracket or tube.

These two forces are referred to as (activational) forces and, once fully inserted, result in (deactivational) forces.

Sometimes these forces are equal and opposite thus resulting in a (pure) moment, when such is the case and equal and opposite moment exist elsewhere with no resulting vertical forces.

However, sometimes the deactivational forces are (unequal), thus resulting in a moment and a force. Static equilibrium requires equal and opposite forces in the same plane of space as their sum must always be (zero).

A simple way to visualize this problem is to engage the rectangular wire into the bracket or tube and while they are still in the passive stage you will note the portion of the wire outside the bracket or tube will lie either gingivally or occlusally on the opposite side.

The longer the moments are permitted to act, the greater the opportunity for canting of the occlusal plane.

Horizontal forces utilizing a round wire can be used for crossbite correction rather than vertical. Twist will no longer be utilized, as is the case with rectangular wire. However, a full appliance must not be in use at the time as bends, not twist, will be activated to produce the moments. Since moments are produced as the result of a force times perpendicular distance to the center of resistance, a horizontal force acting to a tubular bracket will generally lie at a greater distance from the center of resistance than the same force applied vertically to the same tube or bracket.

Equal and opposite forces never again will present the canted occlusal plane problems regardless of duration or age of the patient. However, it is absolutely essential that the activations involved take place prior to placing a complete appliance. If a canted occlusal plane is already present in a full appliance, you may simply place an (overlay) rectangular wire into a removable rectangular tube located on the main arch wire, and then twist the wire in a direction so that the vertical forces created will not be apparent. In other words, if we know that direction of vertical force is causing canted occlusal planes, we simply reverse the direction of these vertical forces to correct the occlusal plane.

None of it is to imply that using TAD is wrong, but simply to point out that fundamentals do not disappear with time, and can save both the patient and the operator money and time.

- Smile curve

Since a smile curve is a function of the occlusal plane, frequently we may have achieved a beautiful curve accidentally. If you simply take a set of plaster models, and outline the upper incisor edges with a red pen, you can rotate the models upward and downward and quite see the effect on the smile curve.

There are enormous areas that could be discussed regarding fundamentals that could eliminate so many unnecessary treatments being done today. Briefly, let me just mention a few. When cusps are finely into a nice Class I relationship during orthodontic treatment, generalized spacing of the anterior teeth can frequently be eliminated by simply diverging the root of incisors, as the increases the mesio-distal requirements. The prescription appliances cannot do this as the wire-bracket analysis will show you that you cannot obtain pure moments on the four incisors where they continue its arch. However, with such a connecting only at the center of incisors, any bypass connecting the lateral incisors, true moments can easily be accomplished. This same approach can be used to eliminate (diastema) without performing frenectomies or circumferential fibrotomies.

A partial appliance, when fully understood, will do many things that were considered impossible. Many midlines can be corrected by molar rotation only. Normally positioned teeth are permitted to remain in the (neutral zone), which means that maloccluded teeth must be treated to the normally positioned teeth. The neutral zone involved a normal functional occlusion with all the forces in balance. As a result of developing a great level of appreciation for the force system that can be developed by partial appliances, I have for many years removed arch wires in 100 percent of my cases for a minimum period of six weeks.

Obviously, there were always those cases where teeth cannot be placed into the neutral zone. But, it seems totally illogical to prevent teeth from enjoying the opportunity for its (stability). There are so much more to be said regarding the appreciation for fundamentals that do not change with time.

In 53 years of practice, I have never used a crossbite elastics, a TPA or a lingual arch. Please do not interpret this statement to mean they should not be used, but a fundamental fact, if applied, can help you avoid of the many of the expensive appliances being promoted today while, at the same time, requiring the patient to simply keep your appointments and brush your teeth.

I consider orthodontist to have been a God-given-vocation. We all know the saying if you love what you are doing, you will never work a day in your life. Enhance your vocation in orthodontics by asking yourselves “(Why   )”.
